2003 Chevy S-10 Code Po128B

I assume this to be the thermostat or coolant temp sensor. Could someone help with an opinion and some help locating each one? Thanks.

Yeah, looks like itis the coolant sensor out of range.I would replace thermostat too, just to be sure.Thermostat is under the housing where the upper radiator hose mounts on the engine Coolant sensor is located on the drivers side cylinder head between the last two cylinders (see pic)
