2001 Chevy S-10 violent shaking between 15-30 mph

my truck violently shakes between 15-30 mph then gets smoother, sometimes it gets shaky again at higher speeds but not as violent.

It sounds like an unbalanced tire or there is some mud build up on the inside of the wheel. Tire weights can and do fall off. Or you could have something much worse and dangerous -- a broken steel belt inside of your tire.
It also could be a loose or worn suspension part but you should look at the previous mentioned first... Let us know what you find...

Check the truck drive shaft, bearings might be wearing out
