i replaced lower ball joint 3 months ago,i heard some clicking and the nut striped off and left me dead in the road i got a new nut a put it back on the threads on the bolt were still good but it is making a clicking noise again,what could be making the nut to strip off

improper installation, not torqued correctly and cotter pin not installed.......
