My mother's car has 3 or 4 times in past year began knocking heavily when started. It will knock the entire time it is running until turned off. Then it may not knock again for a period. Twice she had engine crankcase flushed, new filter and oil installed. No problem for a period then reoccurs. She drives only couple 4-5 times per week and only short distances. talked to local mechanic who only thought is maybe trash or sludge being picked up in crankcase screen, requiring removal of oil pan. Any thoughts?

Maybe carbon deposits causing detonation which sounds like a knocking. A product called Sea Foam is a good cleaner.
