2004 Chevy Blazer irregular brake wear

I have had break classes and ran a grage for several years, but have never seen brakes like this wear so wrong. It's almost like something is completely out of alignment. Part of the pad had 1/2" of pad left and other parts were metal to metal. I took everything apart at 58,000 miles and cleaned and used synthetic brake grease to lubricate all contact points. I even used the ceramic brake pads, had the rotors resurfaced (which were warped beyond belief) It appears to be wearing funny again. I have done several brake jobs over the last 15 years, and I can't figure this one out.
