1989 Chevy Blazer rear differential

i just put on a new cover for my rear differential and drove it around for about an hour. it just blew a hole in it again for the second time. that was the reason i replaced it. what is the problem and how much will it cost to fix

what do you mean by blowing a hole in it. is the correct one being put on. does the rear diff make noise when driving
