2001 Chevy Astro Tuneup or what?

When its cold out, whenever I come to a stop the whole van shakes and the engine will shut off. Also sometimes when driving it'll jerk...like someone is riding the brake. I dont know if this needs a tuneup, tranny flush, or something worse. Do you know what might be wrong?

Clean out the idle air control valve and EGR valve then test the throttle position sensor and manifold absolute pressure sensor.
