It is making a loud ratcheting sound at the top position, and also at the halfway down position where the window will no longer open any further.Like a gear is slipping. Is this the regulator or the window motor? Is it necessary to replace both anyway with a car of this age?

It does sound like the window regulator. Now as for replacing it I would check price. Sometimes it don't cost much more to replace it as a unit with the motor. It is also easier to replace it as a unit too.
