Car acts like the battery is dead after I drive it a few miles

Car starts and runs fine when it's cold and after I drive it for about five miles it wont start or crank like the battery is dead.

Does it return to normal after a period of time? When this happens, do the light work and other component, and the starter fail to engage to start the engine? Are you able to drive it more than five miles without issue and the problem only return when trying to restart it?

If you can think of any other issues related to this that may help, please list them.
