brakes go to floor

when i drive my brakes go to the floor and i have been having to put brake fluid in and when i open hood the hose (bolt ) connected to master cylinder looks like it leaks or is loose. what is the problem?

The seal at the back of the master cylinder (near that bolt) is leaking. You will need to replace the master cylinder. Should not be too expensive, and just about any shade tree/backyard mechanic should be able to do it for cheap.
