brake fluid squirting out of brake reservoir

Replaced all brake components front and rear, master cylinder and brake booster on my van listed above e150, but i have a very soft pedal that goes to the floor when engine is running. also fluid is squirting out of the reservoir when the cap is off. i have bled the system a bunch and at all four wheels its a steady stream of fluid with no air being present. is this a bad master cylinder?
