Boiling coolant reservoir

Lately I have been working on my car but for some reason after driving it for a while and I have my car turned off you can hear the coolant reservoir tank boiling. I have to keep adding coolant to the reservoir tank at least once a week. I tried bleeding the cooling system, changed thermostat, and sensor but I cannot pinpoint what the problem is and the cause for the reservoir tank to boil. Can someone ease enlighten me with some wisdom on fixing this problem? Thank you so much in advance.

Hi,

When driving the vehicle, does the temperature gauge indicate it is overheating? I ask because it could be combustion air into the cooling system.
