Belt Tensioner Installation instructions needed?

My old belt tensioner busted off somehow while driving. I need to install the new one, but I'm not sure how to install it and if I need any mounting bolts or anything. This is the new one:

Wow, usually they lose tension. Did it physically break?

As far as installing the new one is concerned, it isn't too difficult of a job. If you look below, I attached the directions. Take a look through them and let me know if you have questions.
