Belt n pully problem

Belt keeps coming off. Alternator and crank shaft pully look offline about quarter of an inch

From each other or the other pulleys

The two seem to be one or two threads off from the other pullies

Well I would not think both would be off that would be strange but if I had to guess I would thing your balancer is coming apart. There is a rubbed peace in the balancer that's sometimes wears out you can see it if you look close the balancer should be flat on the outside with no lip anywhere.
