Axle replaced and slips even with new axle

Replaced axle because of clunking noise. Car drove fine otherwise and fully operational. Axle replaced and failed. Retaining ring missing when removed. New part installed and retaining ring visibly present. Axle failed again. New mechanic said ring missing when inspected. What could be causing axle failure and ring to go missing? Also, appears repair not necessary as clunking noise still present when axle was working. New mechanic says ball joint worn. Car is not operational due to axle issue. Thanks for any advice!

Yes, that is strange. Can you tell me if it is the front or rear axle you are having trouble with?

It is the passenger side front axle that was replaced. I was told the old axle could not be reinstalled because the end was sheared off. Is it possible there is shards in the transfer case affecting the new axle making the retaining ring come off and making the axle slip out?

Not really, the axle must able to move horizontally causing the axle clip and joint end to shear on come off. Has the car been in an accident?
