1998 Acura Integra Smoking at high RPMS/Burning Oil

I have a 98 Integra GSR with 130k miles that burns through a quart of oil every few days to a week. Also when I rev it past 5000 RPMs it sends a lot of smoke out of my Exhaust. Any help would be gladly appreciated.
