Hesitation/vibration during high torque acceleration

When on an on ramp or going uphill around 40-60 mph.

There seems to be a hesitation and vibration giving the sense that something is "slipping"

Any known issues/problems?

Clearly when we get a hesitation or slipping like this, we suspect a transmission issue. However, Ford had an issue around this time of low fuel pressure causing a hesitation. Volvo shared a lot of common components around this time due to Ford buying them in 1999. So let's start with checking fuel pressure and go from there.
