2009 Daihatsu Terios Hi Clutch Began Slipping Fit Fourth Less Than Year Ago

Hi, the clutch began slipping in fit, then fourth less than a year ago we we replaced it. Eight months later the slippingn has begun again and is much worse this time, slipping in third and second. This occurs when accelerating, generally at speeds over 70km. Any help/suggestions would be much appreciated.

improper adjustment, bad pressure plate, overheated pressure plate from slipping the clutch or a bad flywheel
