What makes the car shake while driving?

The car shakes sometimes. Traveling usually in excess of 50 mph. stops if I slow down. had tires rotated and balanced. Still does it.

Bent wheel. Scale or other debris stuck between the wheel and rotor or between the rotor and hub. Broken tire belt. They don't always cause an easily-identifiable bulge.
