car sometimes jerks/jolts rather hard

I was wondering why my car sometimes jerks/jolts rather hard when the rpm meter hits 4rpm or higher. Any ideas? When accelerating to 4rpm or higher, my car seems to jolt or jerk. What might this problem be?

scan for codes and check fuel pressure first. try cleaning the throttle plate on both sides with choke cleaner as well.
