Ford Taurus X - What causes the cooling fan stays on even when the car is not warmed up?

After starting the car, the fan will start to run continuously. the only time it doesn't run sometimes is the first time you use the car on very cold (-8C)days. After this, even if the car is off for 30 minutes and has cooled down somewhat, the fan will run when the car is started. An engine temperature sensor/relay was changed with no results.
