What are the symptoms and how is it failing? There should be two fans, one belt-driven and one electric. The electric fan is not too likely to even turn on in just five miles unless it's really hot outside and you're driving in the city. It would be much more common to have a grounded wire feeding the fan. That would cause the fuse to blow, but that would have nothing to do with the fan motor itself.
