After letting my car warm up for 5 min. As I'm driving at no more than 20mph under light acceleration my rpm gauge shoots up radically and have to let off so it can keep moving but not without some sputtering/jerk and hesitation. Goes away only after driving a short distance and increasing speed steadily.

Has the check engine light come on? Have you checked fuel pump pressure? When the RPMs go up, does the vehicle accelerate or does it feel like the transmission is slipping?
