antifreeze leak

My car is leaking antifreeze out of a hose that is shaped in a 90 degree. the hose runs from the cylinder head to the water pump.when pressure builds up, antifreeze sprays out of this connection, and the engine runs hot. What needs to be done to correct this issue?
