2006 Jeep Grand Cherokee Engine surge hesitating uphill

While driving uphill my vehicle surges at around 35 mph. It can feel pretty jerky. It sounds like the engine revs up and down as the vehicle surges. As soon as I give it more gas pressing down on the gas pedal harder it will no longer surge. I have replaced the spark plugs, pcv valve, tps, cleaned throttles body, replaced air filter at the same time. The vehicle also has a hard downshift into first grear approaching a stop.

clean throttle plate on both sides with choke cleaner then check for a vacuum leak hos eoff,broken etc. then scan for codes and check fuel pressure with a gauge auto parts rent it.
