does it only blow cold air at idle when the truck is on high idle or driving it is fine

I'll assume you are refering to the heater and not the cooling system. Check to make sure the coolant is full. Make sure the engine is running at a normal operating temp. Start the engine, allow it to warm up with the heater on. Feel both heater core hoses. Both should be hot. IF they are, check the blend door near the heater core. If only one is hot, try flushing the heater core.
