intermittent heater blower motor operation

the blower motor may or may not turn on with the switch in any position. After driving for a bit it may turn on, or hitting a bump and may not reach the selected speed for a while if ever.

Is it most likely the blower motor is failing?

Could be and also the blower motor relay/fan condtrol module and the blower switch
