hard starts and black smoke

i have a 2006 Chevy silverado 4.3 v6 it struggles to start and when it does it shoots out black smoke from the muffler, and u have to push the gas pedal to make it start I have already changed the air filter, and used a gas treatment on it lucas and it didnt work what else could it be?
