There is a humming noise, akin to tire road noise, coming from the rear of my 2005 Lexus GX 470 with 91,000 on it. I replace the old tires with new Michelin's and the noise continues as before with no change. Any ideas?

you can hear this noise only when its moving?, then you probably have a bad hub assembly(bearings),common problem, as long as the tires are good and not rubbing, bearings are most likely the case

Yes, only when moving starting around 20 MPH. Should both rears be replaced at the same time?

its not necessary, however, if you have the funds to do so then go ahead, but we need to make sure this is the problem, try to avoid going to dealers, they are just too expensive.. a local shop should suffice, also they should be able to confirm the problem.
