How do I fix this? No matter if my car is in drive, reverse, or park the gear shift indicator does not move. The indicator is always on D. I am able to shift in to gear just fine. I just have to be sure to shift down one gear to be in reverse, etc. I had someone look at this a couple of years ago when it happened the first time. He took apart the stearing column and did not find any broken or worn parts. He told me that there was a lot of dirt and debris in the steeling column. He said he cleaned it up real well and it should be fine. It lasted for over a year working just fine. Now it is not working. What is wrong and how do I fix it?

Chances are the control wire is hang up or the plastic holder is broken. That is the first thing I would check. Remove the outer casing around the steering column and check for dirt. Something is most likely causing it to hang up.
