2005 Chevrolet Colorado Clutch Seems Engage Disengage Top Also Sometimes

My clutch seems to be engage and disengage at top..also sometimes driving the car jerks like its missing is this the clutch slipping .

if you have the proper free play for the pedal, yes the clutch most likely needs to be replaced.
