Shaking at 68-78 mph after tire rotation/brake pad replacement

Lexus RX 330, 2004. Steering wheel shaking between about 68 and 78 mph. Just had the tires rotated. Also had the front brake pads replaced and they machined the rotors. What's the problem? I've always rotated my tires every 5,000- 6,000 miles.

This has nothing to do with rotating tires. Shaking is related to tire balance, a broken belt, or if the problem started right after the brake work, it could be a warped rotor or rust or other debris between the rotor and hub. The most common cause is a tire balance.
