Clunking noise coming from gearbox when changing gears or applying brakes on 2004 jeep cherokee sport 2.4l

The brake discs need skimming because when applying brakes the car "shudders". BUT during this shudder a clunking noise is heard in the gearbox (metal on metal) and this happens especially in higher gears (4&5). What could it be? Oil level is fine.

first of all, the shudders is the rotors out of round. this vibration and thumping noise could be associated with the gear box. get this fixed first and see if the problem persists.
