My car jerks while accelerating from stop

Hi, my car jerks while accelerating from stop. Jerks every 500 rpm starting from 0 to 2000. Does not happen when i put it in slap shift on second gear and higher. What is wrong?

scan for codes and clean throttle plate with choke cleaner on both sides and IAC hole. finally check fuel pressure with a gauge auto parts rent it.
