2003 Nissan Xterra acceleration problems

Whenever I am driving down the road and I let off the accelerator momentarily, the vehicle feels as though the drive train has been disengaged, then it lunges forward whenever i touch the gas again.

scan for codes and make sure trans fluid is full an dnot dark brown or black if it is go righ tto a trans shop
