2003 Holden Commodore Car surging

I have a vy Holden and when I go to take of with a little bit of speed it starts to surge till it gets to about 50ks than comes good even if I hold my foot on the break and put my foot down as if I was trying to spin the wheels it surges and goes to stall I've put injector cleaner in it and have changed the fuel filter but Otis still doing the same thing. Reverse and neutral work fine
