engine cut out choke up and low revs ,with grey smoke !

Ford transit on a 53 plate .some times it will choke up at 200 revs and chuck out grey smoke for little while then be OK .and some times looses power !

Have changed fuel filter!and agr valve was cleaned but no change!

check fuel pressure and back pressure on the cat converter
