Shifting from drive to reverse and reverse to drive there is a hard jerk

When i turn off ignition it stops, this happens about 2 to 3 times a month. The car shifts smoothly when on the road and the transmission oil and filter have been changed recently. Is this a transmission issue. Thanks for the help...

Sounds like an engine mount issue allowing the engine or transmission to become loose causing the jerking motion.
