2002 Ford Windstar High Idle then dies

My 2002 winstar Idles at 3500 rpms then Idles down to around 1100 rpms then dies, when I restart it, it high Idles at 3500 rpms then Idles down to 1100 rpms Idles rough then dies, but not every time, sometimes it Idles at 1100 rpms rough and then dies.

Clan throttle plate on both sides and iac hole with choke cleaner and check for a vacuum leak, like hose off leaking etc.

Yep, this sounds like the PCV valve hose has a leak which is common, here is a guide to help you find the problem and I have included images below.
