Why does my rear widshield washer doesn't spray right?

When I try to clean the rear window the washer fluid doesn't spray it just comes out in a stream, I have cleaned the nozzle with a toothbrush but still nothing. How do you change that type of spray nozzle?

Rear nozzles usually don't come in spray patterns.
