Flex plate fracture in transmission

I have 01 toyota tacoma 2 wheel drive prerunner truck. problem is flex plate fractures around bolt pattern where flex plate attaches to crankshaft.

The flexplate is a stamped peice with a welded gear ring. If it has failed more than once, I would ask where are you getting the replacement, autoparts store or Toyota?
