can break a valve and drill the engine cylinder 2JZ GE being in power without accelerating in park?

my car had a fault in the coils and the mechanic told me that at the end of change that when doing the tests having the engine running without speeding it broke a valve and the cylinder is drilled.
why did this happen?

sometimes stuff happens and does this. it may have had a piece come out while doing testing.
