My car continues to accelerate when my foot is taken off the gas, what is causing this?

I noticed a few times now, that when I am driving my car, I can take my foot off the gas and the car continues to accelerate. If I put the car into neutral it idles at about 4 grand, this also happens when I put it in park. When I break, I can tell the brakes are struggling to stop the car as well. This has happened about 3 times in the past week.

Scan the computer for code/s and also clean out the idle air control valve and throttle body and see what happens-
