'00 Plymouth Neon transmission?

I don't know what is wrong with my car! It starts up fine, but the problems come when I put it into reverse or drive, the transmission seems to "slip". It won't accelerate properly and after having to rev the engine it seems to lunge forward, also when driving it sometimes seems to stall when accelerating into the next gear. My car is a 2000 Plymouth Neon 4 door. can anyone tell me what's wrong with it?

Check tranny fluids
Check for codes
Hold trip button down cycle ignition ON OFF 3 times ending key on (engine OFF) release trip button
Read codes in odometer area

When I check the fluid, it is empty I just refilled it about a week ago and I am having problems with it again. no check engine light is on and when I put fluid in it, it works fine for about a week, then I have problems with it again. this just started maybe a month ago, so I am hoping it isn't something that costs alot of money to repair.
