car losing power uphill

my car is driving perfectly on flat ground but when i accelerate uphill its losing power and almost cutting out, same when on motorway, fine at 3,000 RPM but when you take your foot off from it it struggles to get back up power / speed. Can you please tell me what may cause this problem?

scan for codes and check fuel pressure. we dont' have colts of that year in u.s. but all models is given for spec and it's 38psi w/vacuum on regulator at idle and 47-50 w/o
