When coming to a stop or pressing on the brakes my car shakes tremendously, is it the brakes?

While driving you can hear rattling over every bump. When slowing down or coming to a complete stop the car shakes tremendously. If I was to slam on my brakes, I don't the car would stop because it shakes to bad.

the rotors are warped from overheating. replace the pads and rotors and be gentle on the brakes.
