How to diagnose transmission problems.

For the last 3 years I have had no first gear in my truck. When in first gear it just acts like the truck is in neutral. I have put on about 40k miles on it with this problem. Everyone I ask keeps pointing me in different directions but none have been helpful. What could the problem be. I am not a tranny guy at all, so any help would be great.
