drive axle has come off while driving

I have a 2000 Chrysler LHS. I replaced the driveaxles and one has come off the transaxle while driving. It has done this two times. I replaced the driveaxle with a new one but it did the same. What could be causing this?
I have had one the new ones to to come off the transaxle while driving. I replaced it with another new drive axle and it did the same thing. What could cause this?

do those snap lock into tranny? was there a problem with old axle coming out? did you check to see if axle was identical length with old 1?
