2000 Chrysler Cirrus wont stay in gear

I have a 2000 chrysler cirrius with 80000 miles with a automatic transmission. WHen in gear driving it will come out of gear i think. You will be driving and all of the sudden the rpms jump up and thenthe car jerks and rpms come down.

Make sure the trans fluid is clean and full. If the check engine light is on, have the computer scanned to trans trouble codes.
