1999 toyota 4runner dead engine

I have a 1999 toyota 4 runner. i do regular maintainance and my engine blow out. A rod came by the passenger side. Although i change my oil every 3000 miles. Which are the possible reasons for this to ocurr.

You need to inspect the damaged components to determine what caused the rod to blow.

I have seen some that are due to con-rod bearing cap bolts breaking. Others are due to oil filter failure or not properly tightened resulting in oil drying up.
